We provide high quality, on-site care for elders in Assisted Living, Long Term Care and Skilled Rehab. Our group of geriatric specialists is small and selective with a focus on providing extended presence in facilities and expert care to complex patients.

What We Do

Expert Care

We are a small group of providers chosen for their experience in long term care. Our providers all have at least 3 years’ experience in working in long term care, some of our providers have over 20 years.

Extended Presence

We are committed to giving each facility and your residents our undivided attention. Our providers pledge to spend at least 3-hour blocks of time for each visit to a facility. We are available 24 hours a day, 365 days a year via a nurse’s hotline which connects you directly with a provider!

Geriatric Care

The service of our elders is a great honor – we take this seriously. We are driven to provide the best health care available to our patients.

Assisted Living

We are strong supporters of autonomous Nurse Practitioners. Each assisted living facility will benefit from at least weekly visits from an autonomous Nurse Practitioner. They will also have the support of an experience physician as needed for ongoing advice with complex patients.

Long Term Care

We pledge daily presence of either a physician, nurse practitioner or physician’s assistant, on time completion of recertifications, low antipsychotic and benzodiazepine use and Saturday rounding by a NP or PA.

Skilled Rehab

We pledge daily presence of a physician, nurse practitioner or physician’s assistant. Our physicians pledge to substantial presence (at least 3 hours), 3 times weekly, in each Skilled Rehab Facility.

Meet Your Team

Dr. Jim Wright

MD, PhD, MATS, CMD

Dr. Wright received his PhD and MD from VCU Medical School in Richmond VA, and holds a Master of Arts in Theological Studies from Union Presbyterian Seminary, as well as a Certificate in Bioethics from Loyola University, Chicago. He specializes in geriatrics and is medical director of two facilities in Richmond, VA, Our Lady of Hope and The Memory Center.

He is widely published in a number of peer reviewed journals including The Journal of the American Geriatrics Society, and The Journal of the American Medical Director’s Association. He has a particular interest in improving quality of care and quality of life for those living in long term care and assisted living settings.

Tangela Crawley-Hardy

MSN, APRN, AGNP-C

Tangela Crawley-Hardy is an Adult Primary Care Geriatric Nurse Practitioner with Autonomous Practice Authority in the state of Virginia. She has been in long-term care and geriatrics for over fifteen years in various positions as well as Hospice and Palliative Care since 2005.

Tangela completed undergraduate work at both John Tyler Community College and Virginia Commonwealth University and went on to get a Masters Degree in Nursing from Walden University in 2015. She is board certified by the American Association of Nurse Practitioners and a member of the Geriatric Advanced Practice Nursing Association (GAPNA).

William Reed

MD, CMD

Dr. Reed is Medical Director of Westminster Canterbury Richmond. He has more than 20 years experience working with the elderly in skilled rehab and long term care as well as a wealth of experience in hospital medicine. He obtained his undergraduate degree from the University of Virginia, and went on to earn both his Master’s degree and his Medical degree from Virginia Commonwealth University, graduating in 2001. He was a founding member of the Henrico Doctors Hospitalist team where he worked for 10 years. He is board certified in Family Medicine and specializes in geriatrics and palliative care.

Bre Becht

MMS, PA-C

Brianne Becht is a nationally certified physician assistant specializing in geriatric medicine since 2013.  She completed her Bachelor’s degree at The Florida State University and graduated from Nova Southeastern University, Florida with a Masters in Medical Science in 2009.   She spent four years practicing internal medicine with a focus on hospitalized patients where she developed a specific interest in the underserved elderly. Upon relocation to Virginia she found a small geriatrics group that shared her passion.  She now has the privilege of serving as an advocate for the geriatric population and our Veterans.

Dr. Arleeta Diggs

MD

Dr. Arleeta Diggs is a board certified family medicine physician who is a lifelong local. She received her bachelors degree from Howard University in Washington DC. Her medical doctorate degree was obtained at Eastern Virginia Medical School in Norfolk, Virginia. She completed residency at VCU/Hanover Family Physicians in Mechanicsville, Virginia. She is a proud wife and mother of six children (yes SIX) and a weight challenged cat who is approximately the size of a small child.

Lynn Simpkins

FNP, GNP

Lynn Simpkins is a Family and Geriatric Nurse Practitioner with Autonomous Practice Authority in the state of Virginia, and is board certified by the American Association of Nurse Practitioners and the American Nurses Credentialing Center. She graduated from the University of Virginia in 1995 with a postgraduate certification as a Family Nurse Practitioner, and later completed training as a Geriatric Nurse Practitioner. She has worked in long-term care and geriatrics for over 15 years.

Lynn holds a postgraduate degree in Advanced Practice Hospice and Palliative Care from the University of Colorado and is boarded as a Hospice and Palliative Care Nurse Practitioner. In addition she is a Certified Diabetes Educator, a Certified Dementia Practitioner and completed a postgraduate degree from West Virginia University in Emergency Medicine. She is passionate about individualizing care for geriatric patients based on their goals of care.

When she isn’t working, Lynn enjoys knitting, and long hikes in the mountains with her husband and dogs.

Laura Kreisa

NP

Laura Kreisa relocated to Richmond after completing her BS in nursing at the University of Delaware. She had the opportunity for various experiences across the continuum at MCV (now VCU Health), including critical care, cardiology, outpatient primary care, heart failure & transplant, leadership positions, and performance improvement. She earned her MS at VCU – FNP (Family Nurse Practitioner) during that time.

Over the past six years, she has partnered with residents in Assisted Living Communities in the Richmond area as their primary care provider.

She enjoys playing the piano, reading, and escaping with her husband to their condo in VA Beach.

Padmalatha Dharanikota

MD

Dr. Dharanikota is Medical Director of Autumn Care of Mechanicsville. She has decades of experience working in long term care and skilled rehabilitation as well as extensive experience in hospital medicine at Chippenham and Johnston Willis Hospital. Dr. Dharnikota received her medical degree from Andhra Medical College, Visakhapatnam, India and completed her residency in Internal Medicine at McLaren Regional Medical Center, Flint, Michigan.

Erica Carter

MD

Dr. Carter received her MD from VCU School of Medicine and completed her residency in Family Medicine at Carolinas Medical Center Northeast. She has over a decade experience in Hospital Medicine and has been working in the skilled rehab field for over 3 years.
